I hereby invest authority in The Nottingham School to discipline my child as necessary. I further agree that I will cooperate with the school and discipline my child in the home as needed.

I do pledge my fullest cooperation to keep doctrinal controversy and denominationalism out of the school at all times.

I agree that if my child should become involved in any trouble at school, or if I disagree with any policies set by the school, I will in no case complain to any parent, but in the love of Christ and in fervent prayer, I will only register any necessary complaints with the teacher and/or principal in private.

I also agree to pay the tuition and fees according to arrangements that shall be made, and to conclude all required payments on or before the due date. I understand that failing such payment, the student will be denied admission to classes.

Liability Relief

I give permission for my child to participate in all school activities, including sports and field trips, with the exception of those listed below. Further, in the event my child becomes ill, or is injured while under school supervision, I approve of the school authorities taking the following steps:
A. Contact a parent or guardian of the student and follow his/her instructions
B. When no parent or guardian can be reached, contact the student's physician and follow his/her instructions.
C. When no parent or guardian or the student's physician can be reached, contact a properly licensed practicing physician and follow his/her instructions.
